SOCIAL WORK SUPPORT AND PRIVATE CASE MANAGEMENT SERVICES assist clients and their caregivers with obtaining the resources and information they need to live their lives with dignity. This includes locating and applying for viable community resources and government programs and benefits based on eligibility criteria (e.g., food stamps, Medicaid, rent assistance programs, patient assistance programs for affordable medications, meals on wheels).
PRIVATE CASE MANAGEMENT alleviates caregiver stress and overwhelm or provides oversight when a caregiver is out of town or cannot provide the services due to health and other reasons.
Social workers provide and research community resources which help patients and caregivers cope with illness and disability and may alleviate financial stress if the applicant qualifies for financial-assistance programs.
Social workers may also assist with-advocacy (e.g., fighting for customer rights)-acting as a liaison between the client and the services they already receive-provide caregiver support-educate and review advance directives such as health care proxies.
